If the item exists in the catalog, the "Type of Request" field should be pre-filled based on the metadata in the catalog record.
Figure out what url parameter in the button urls matches up to the 'Type of Request' form element in the destination form(s) and add that parameter and the appropriate value to the button url.
If the value in the catalog entry is the same as the value for the form, this will just be a configuration change through the UI.
Otherwise, we might have to do some magic to translate the relevant value in the catalog entry to the value needed by the destination form.
