Task Brief

This change is on the page when an admin adds/edits an item.
On the old website, if an item is marked as identifiable, two fields specifying "identification" and "owner notified" pop up. We want to add these fields (with the same placeholders) to the new website.

Expected Deliverables

Add two new fields "Identification" and "Owner Notified" to the new website, exactly how it shows up on the old website (they should be required fields if "Identifiable" is marked. In terms of positioning, they should show up right below the "Identifiable" check box.
