Remove unneeded Index section from UD templates article (#21)

The Index section was a two-item mini-TOC whose links (`the-why` /
`the-how`) 404'd as relative pages. Rather than wire up in-page anchors,
remove the Index block entirely — it added no value ahead of two adjacent
headings.

Closes #21
