We now index abbreviation long forms on the position of the short form, i.e. "emergency room" on "ER". The issue is that both long form tokens get a position increment of 0. This is the intention but has the effect that phrase queries à la emergency room don't work since we cannot match the exact sequence but need to allow a phrase slop of 1 since room is "shifted" one position to the left (some position as emergency). I do not know if there is a good solution to this, do some research.
