Thanks for preparing this! I'd suggest including some (potentially non-normative) mention of the performance advice at https://github.com/smart-on-fhir/smart-scheduling-links/blob/master/specification.md#performance-considerations .
Specifically would recommend that servers return etags and support etag-based If-None-Match, as well as time-based If-Modified-Since. Most cloud file hosting services do this out of the box, I think.
