Map advance dosn't allways work

When a map ends sometimes, it doesn't advance, even though it says it does

I've had this problem for a few months now...

Ok. Can you figure out what's causing it (or under what circumstances it 
happens)? It will be hard to troubleshoot the problem otherwise.

I don't think i possess the knowledge to do that. 
I can give you log entries when it happens,
but nothing seams out of the ordinary, no trackback or anything...
