User Story

As a user of BDT, I would like to be able to name an instance of an EligibiltyCheck that I add to a Benefit within a Screener, so that I can organize my checks and differentiate between multiple instances of the same check.

Acceptance Criteria

• On the Configure Benefit screen, there is a way to name an EligibilityCheck added to a Benefit.
• This "alias name" defaults to "{check_name} - {version}".
