Reconcile stale agent failure ledger projections

Context

PR #1491 deliberately records the repeated command-composition failure as open: a truthful resolution entry can only be appended after the guide is merged and #1490 closes. Its adversarial review also found that leaving that future write untracked would close the only durable tracker.

The existing failure ledger separately still renders three resolved defects as open:

This is a bounded ledger-triage slice of #1291. It does not close #1291 and does not authorize checking any maintainer-owned item in OUTSTANDING_TASKS.md.

Dependency

• Do not implement until PR Document safe PowerShell command composition #1491 has merged and Document safe PowerShell and native command composition for agents #1490 is closed.
• Branch from the resulting main.
• Draft PR Fail closed on incomplete project priority audits #1488 touches the same append-only JSONL/rendered files; it remains data-policy blocked and must later absorb the new main rows, regenerate Markdown, and rerun all exact-head gates.

Scope

• Append one agent/tool-command-composition resolution record using the same surface and Document safe PowerShell and native command composition for agents #1490 projection key after Document safe PowerShell command composition #1491 merge/Document safe PowerShell and native command composition for agents #1490 closure.
• Append resolution records for [Bug] Prevent late summaries from overwriting workspace mode #1343, Paper Review crashes on numeric deep-review enum payloads #1347, and Similar-past endpoint returns 500 for real SQLite proposals #1348 without deleting or rewriting their original JSONL history.
• Keep [Bug] Prevent late summaries from overwriting workspace mode #1343/Paper Review crashes on numeric deep-review enum payloads #1347/Similar-past endpoint returns 500 for real SQLite proposals #1348 first in their respective future_fix values so the renderer projects the latest state.
• Preserve the raw seed record but suppress class=seed from rendered output.
• Keep the unrelated ci/e2e-smoke invalid-signal timeout open; broad later green E2E is not focused root-cause proof.
• Regenerate docs/agentic/FAILURE_LEDGER.md.
• Do not update product status/roadmap docs; shipped product truth is unchanged.

Acceptance criteria

• The Document safe PowerShell and native command composition for agents #1490 command-composition record renders exactly once as resolved after its dependency is satisfied.
• [Bug] Prevent late summaries from overwriting workspace mode #1343, Paper Review crashes on numeric deep-review enum payloads #1347, and Similar-past endpoint returns 500 for real SQLite proposals #1348 each render exactly once as resolved, with qualified evidence that does not erase residual debt.
• Original JSONL rows remain byte-preserved; only append-only resolution rows are added.
• Raw seed history remains present but never renders, including seed-only input.
• Non-seed issue-less entries remain visible; ci/e2e-smoke remains open.
• Renderer regressions pin projection and seed suppression.
• py -3 -m unittest test_render_failure_ledger.py passes before and after regeneration.
• node scripts/check-docs-governance.mjs, check-golden-principles.mjs, and check-github-ops-governance.mjs pass.
• Rendered Markdown is byte-identical to a fresh renderer run and git diff --check passes.
• No unrelated failure status, docs/STATUS.md, docs/IMPLEMENTATION_MASTERPLAN.md, or human checklist item changes.
